By participating in this Zumba class, I acknowledge that I am voluntarily engaging in physical exercise and dance fitness activities that involve inherent risks. These risks may include, but are not limited to, muscle strains, sprains, falls, dizziness, dehydration, or other physical injury or health complications.
I understand that it is my responsibility to participate safely, listen to my body, and modify or stop movements as needed. I understand I should consult a physician before participating.
I voluntarily assume all risks associated with participation in this class, whether known or unknown, and accept full responsibility for any injury or health issue that may occur.
I hereby release, waive, and discharge the instructor, facility, and any affiliated parties from any and all claims, liability, damages, or expenses arising from participation, including claims based on ordinary negligence.
I understand that I am responsible for any medical treatment or expenses incurred as a result of participation.
By checking the agreement box and participating in class, I confirm that I have read, understood, and voluntarily agree to this waiver in full.
